I was going to describe those changes as temporary, to be undone after
applying unifdef, but it turns out they're both in dead code, so there
will be nothing to undo after unifdef has run.

Remove mentions of MBEDTLS_KEY_EXCHANGE_RSA_PSK that were not guarded by the
configuration option MBEDTLS_KEY_EXCHANGE_RSA_PSK_ENABLED. This finishes the
removal of library code that supports the RSA-PSK key exchange in TLS 1.2.

Remove the configuration option MBEDTLS_KEY_EXCHANGE_RSA_PSK_ENABLED and all
code guarded by it. This remove support for the RSA-PSK key exchange in TLS

The endpoint field is needed to serialize/deserialize
a session in TLS 1.2 the same way it is needed in the
TLS 1.3 case: client specific fields that should not
be in the serialized version on server side if both
TLS client and server are enabled in the TLS library.

GCC warns about comparing uint8_t to a size that may be >255.
Strangely, casting the uint8_t to a size_t in the comparison expression
doesn't avoid the warning. So change the type of the variable.

In client-side code with MBEDTLS_USE_PSA_CRYPTO, use the buffer size to
validate what is written in handshake->xxdh_psa_peerkey. The previous code
was correct, but a little fragile to misconfiguration or maintenance.

Most functions in psa_util.h are going to end up there (except those
that can be static in one file), but I wanted to have separate commits
for file creation and moving code around, so for now the new file's
pretty empty but that will change in the next few commits.

Now the MD macro also accounts for PSA-only hashes.
Just a search-and-replace, plus manually removing the definition in
hash_info.h.
